Overview
Unifyr One allows you to create your own custom forms for capturing these types of data:
- Partner information - data that will be added to the partner profile (e.g. Partner Management > Partner Explorer > partner > Profile in Unifyr One Admin)
- Partner User information - data that will be added to a user’s profile (e.g. Partner Management > Partner Explorer > partner > Users > user in Unifyr One Admin)
Note
Custom Partner and Partner User forms for the partner portal aren't supported with a Unifyr One Engage subscription.
You can have multiple forms for different types of information. Each form can contain core partner/user fields and your own custom fields, if required.

Note
You may need to contact your Unifyr One team if you want to manage your own partner / partner user forms, as it needs to be done in a controlled way to ensure any custom code for the portal isn't affected.
Before You Begin
If you want to include custom fields in your form, ensure that they’re already set up for use. For more information, see Custom Fields.

Create a Custom Form for Unifyr One Admin
You can create custom forms to categorize your partner or partner user data records in Unifyr One Admin.
Any custom partner forms you set up are presented in the Profile area of partner accounts. For more information, see the View a Custom Partner Form in Unifyr One Admin section of this article.
Your custom partner user forms appear in a partner user's record (accessed either from the Users tab of the partner account, or from Partner Management > User Explorer). For more information, see the View a Custom Partner User Form in Unifyr One Admin section of this article.
- Navigate to > Settings > Forms & Fields.
- Choose the Partner or Partner User category.
- Go to the Forms tab.

Click Create Form, then choose Unifyr One from the dropdown menu.

In Create Form, click the pencil icon next to the title to add a new title for the form. Click Save to save the title.
Note that the label next to the name reads Unifyr One. This form is available for your use in Unifyr One Admin.
- In the Fields tab, Available Fields shows the fields that can be added to the form. Some Unifyr One core data fields, and your custom fields from the Partner category, are listed.

Use one of these methods to add a field to the form:
- Drag and drop the required field from Available Fields directly to the required position in Selected Fields.
- Click the Add icon
to move the field from Available Fields to Selected Fields. The field is added to the bottom of the list - you can drag it to the required location.
Update the Display Name, and the Required, Read-only, and Display Condition properties as required. For more information about the properties, see Form Configuration.

To remove a field, select it in Selected Fields and click the X.
Note: If the field shows a padlock icon, rather than an X, you can't remove it. This field is required by Unifyr One at a system level.
- Click Save to preserve the update, Save and Close to complete it and return to the form list, or Cancel to abandon the update.
Create a Custom Form for the Partner Portal
You can create custom forms to collect partner or partner user data in your partner portal.
Note
Custom Partner and Partner User forms for the partner portal aren't supported with a Unifyr One Engage subscription.
- Navigate to > Settings > Forms & Fields.
- Choose the category of form you want to create. Custom portal forms can be created for these categories:
- Partner
- Partner User
- Go to the Forms tab.
- Click Create Form, then choose Partner Portal from the dropdown menu.

In Create Form, click the pencil icon next to the title to add a new title for the form. Click Save to save the title.
Note that the label next to the name reads Partner Portal. You can add this form to your partner portal so that partners can submit information to you. For more information, see the Make a Custom Partner/User Form Available in the Portal section of this article.

- [Optional] You can add a custom header to the form - for example, some guidance to help partners use it. Go to the Other tab and add the required text to the Help Text field. For more information, see Adding a Custom Header to a Form.

Edit a Custom Form
When editing a custom form, you can make changes to the fields the form contains, or the setup of each field.
- Navigate to > Settings > Forms & Fields.
- Select the category of form (Partner or Partner User) then go to the Forms tab.
- Click the form you want, to open it for edit.
- Make the required changes. For more information about the changes you can make, see Form Configuration.
- Click Save to preserve the update, Save and Close to complete it and return to the form list, or Cancel to abandon the update.
Make a Custom Partner/User Form Available in the Portal
You can provide a custom form in any of your custom portal pages so it’s visible to your portal users. You can use these instructions to present any of your custom partner/user forms of type Partner Portal.
Add the Custom Partner/User Form dynamic content widget to a custom page in your site navigation to make a form available to your selected partners. You select one form for each instance of the widget. For more information, see Add Dynamic Content.
- Navigate to Partner Portal > Custom Pages.
- Select the required custom page from the list, or click Create Page to create one.
- Edit the custom page to add the Dynamic Content widget to the required area.

Click on the widget and select the Custom Partner/User Form type for the widget.
Tip: You may need to select More Content when searching for the widget.

Complete this information:
Parameter Description Select a custom form type Choose the category of form from the drop down list (User or Partner) Select a custom form Choose the form from the drop down list.
The list shows all custom forms of type Partner Portal from the category you selected.
- Click Insert to add the widget to the page.
- Add other content to the page as required.
- Click Save to preserve the update, Save and Close to complete it and return to the form list, or Cancel to abandon the update.
- Add the custom page to your portal site configuration. For more information, see Adding Custom Pages to a Partner Portal.
Your partner can now access the form from the page in which you provided it. They can enter the information you’ve requested, and click Save to submit it to your records.
View a Custom Partner Form in Unifyr One Admin
Any custom partner forms you set up are presented as individual tabs in the Profile area of partner accounts, in addition to the Partner Profile form. You will see a tab for each custom form of type Unifyr One you have available.
- Navigate to Partner Management > Partner Accounts.
- Choose a partner and click on their name to open their record.
- Navigate to the Profile tab in the left sidebar.
- The tabs across the top of the page show the forms. For information about managing the data in the tabs, see Editing a Partner's Profile and Custom Fields.
View a Custom Partner User Form in Unifyr One Admin
Any custom partner user forms you set up are presented as individual tabs in a partner user account, in addition to the Partner User Profile form. You will see a tab for each custom form of type Unifyr One you have available.
- Navigate to Partner Management > Partner Accounts.
- Choose a partner and click on their name to open their record.
- Go to the Users tab.
- Find the user whose information you want to view, and click on it to open it for edit.
- The tabs across the top of the page show the forms. For information about managing the data in the tabs, see Managing Individual Partner User Details and Editing a Partner's Profile and Custom-Fields.
